Athletic Forms

All students participating in MCMS Sports must have their parents sign them up for an account on DragonFly Max and complete the forms (It is easier to complete online instead of your phone). Students must also get a physical to complete and turn in or scan into the DragonFly app prior to participating for a sport in the 2024/2025 school year.

6th Grade students may participate in the following sports:
Cross Country
Golf
Tennis 
Track and Field
Wrestling

7th and 8th Grade Students may participate in all sports.
